Nurse Care Coordinator, Home Hospice

$8,000 sign-on bonus offered

Hospice care at the Aurora Visiting Nurse Association provides crucial end-of-life support for patients and their families. We focus on maintaining quality of life by keeping patients pain-free and comfortable while providing for spiritual, emotional, social and physical needs.

Home care practice allows you personal flexibility for work-life balance by setting schedules that balance patient needs with your own personal priorities. The home care setting is very rewarding and challenging, allowing you to get to know your patients in a very personal setting and to be creative in meeting their needs.

We are looking for a Nurse Care Coordinator for home hospice services that offers:

  • One-on-one patient care
  • Holistic treatment, including education and extended care of the patient
  • Independence and autonomy
  • First shift, weekday hours for many positions that allow greater work/life balance
  • Job satisfaction from having a real impact on your patient's life
  • Fuller spectrum of health care and illness issues than are typically found in a hospital setting
  • Multi-disciplinary approach to health care that incorporates physicians, therapists, dieticians, social workers, pharmacists and home health aides
  • Aurora VNA reputation for industry leadership and commitment to the patient
  • Ability for advancement within Aurora Health Care, the most comprehensive health care provider in the state of Wisconsin
  • Comprehensive orientation that includes two-six months of classroom time and clinical visits
  • RN clinician resources in specialty areas including wound care, infusion, diabetes, pediatrics, hospice and behavioral health
  • Competitive wage and benefits
  • Quality analyst support staff to complete coding and 485's

Technical skills/knowledge needed

  • Wisconsin RN license
  • Wisconsin Drivers License
  • At least one year of acute care, home health or hospice experience
