Field case management

Aurora has partnered with Encore Unlimited to provide field case management services throughout Wisconsin. This group of highly skilled case managers is familiar with nuances of the worker's compensation system.

Employers spend billions of dollars in workers' compensation costs, time away from work, lost production and medical care each year. Employers benefit from Aurora's field case management service through an expedited return-to-work timeline and consistent information.

When an employee is not at work

The use of field case management controls costs while ensuring the employee can return to work without jeopardizing their safety.

Field case management gives the employer, medical provider and workers' compensation insurance carrier current information to assist in the decision making process. Field case management entails working 1-on-1 with the employee to facilitate a transition from injury to full return to work in a safe and cost effective manner. The field case managers are also in communication with Aurora's internal telephonic case managers, if needed.

Aurora brings the same attention to detail and skill to each case whether the referral is a task assignment, a typical 3-point contact or a catastrophic case assignment. Our goals are to resolve the case as quickly as possible while ensuring that all parties receive the services they need to meet their individual goals.

How do I make a referral to Aurora's field case management services?

You have 3 options:

1. Go to www.encore-unlimited.com/aurora for the online referral form developed specifically for Aurora Occupational Health. Complete this form with the pertinent information and a local case manager will call you within 24-48 hours.
2. If you prefer, you may fax the WC-12 to 800-536-5023 with the referral form provided.
3. Or simply call 800-434-2520 and provide the information to the referral coordinator. Written information on the referral process will also be provided to you.
