Aurora Health Care and UnitedHealthcare to Improve Patients’ Care in Wisconsin with New Accountable Care Program

Program designed to help better manage population health, provide more resources to patients and lower costs

MILWAUKEE (June 22, 2016) – Aurora Health Care and UnitedHealthcare have launched an accountable care program designed to help improve people’s health and satisfaction with their health care experience. The new program will benefit the more than 40,000 people in the Milwaukee area enrolled in UnitedHealthcare’s Medicare Advantage plans.

UnitedHealthcare and Aurora Health Care will work together to better coordinate patients’ care, using shared technology, timely data and information about health care visits to find more effective ways to treat patients. The accountable care program will also provide services to help patients manage their chronic health conditions by enhancing relationships with their primary care physicians and encouraging healthy lifestyles.

Aurora Health Care is an integrated health care provider serving communities throughout eastern Wisconsin and northern Illinois with 15 hospitals, more than 150 clinics and 70 pharmacies in 30 communities.

“Our focus is on helping our people live well, and we know that by leveraging our care management philosophy with population health analytics we can help increase quality and efficiency of patient outcomes while reducing redundancy and health care costs,” said Titus Muzi, Aurora Health Care’s senior vice president of managed care contracting.

UnitedHealthcare will supplement Aurora Health Care’s own data to help support overall population health, providing the entire care team with clear, actionable data about individual patients’ health needs, potential gaps in care and proactive identification of high-risk patients. Patient navigators may also be used to support community-based care coordination, such as helping with planning after a patient is discharged from the hospital and scheduling follow-up appointments.

Through this collaboration, Aurora Health Care and UnitedHealthcare will work together to better coordinate patients’ care under a value-based, patient-centric care model focused on keeping people healthy.

UnitedHealthcare serves more than 1.5 million Wisconsin residents with a network of

150 hospitals and more than 28,000 physicians and other care providers statewide.

More than 14 million people enrolled in UnitedHealthcare plans across the country have access to accountable care programs, delivered in part through more than 750 accountable care arrangements nationwide as the organization engages in deeper, more collaborative relationships with physicians and hospitals.

Care providers nationwide are showing strong interest in a shift to value-based care. UnitedHealthcare’s total payments to physicians and hospitals that are tied to value-based arrangements have tripled in the last three years to over $48 billion. By the end of 2018, UnitedHealthcare expects that figure to reach $65 billion.
