Our community health research
We’ve strengthened our standard of care with rigorous community health research to identify the most pressing health needs across the communities we serve.
This important commitment is anchored by Community Health Needs Assessments (CHNAs), conducted every three years by each of our hospitals, and brought to life through comprehensive Implementation Strategies (IS) that are updated each year.
Community health data
Since 2003, Aurora Health Care has partnered with local health departments across eastern Wisconsin to survey residents on their health status and habits and identify areas of greatest need.
These comprehensive community health surveys, conducted every three years, allow our hospitals to:
- Gather specific data on behavioral and lifestyle habits of the adult population, including selected information about each respondent’s household.
- Accumulate data on the prevalence of risk factors and disease conditions existing within the adult population.
- Compare, where appropriate and available, health data of residents to state and national measurements.
The primary community health data provided by these surveys is supplemented with secondary data and key informant interview reports, rounding out a range of reliable data sources for Community Health Needs Assessments.
Our hospitals
With insights gained through CHNAs, each of our hospitals is responsible for establishing a proactive Implementation Strategy that maps out how they will use their resources to address community health needs.
While hospitals’ specific priorities vary, all demonstrate dedication to Aurora’s three core principles of community benefit:
- Increasing access and expanding coverage
- Providing a community health improvement plan
- Building a seamless continuum of care
