  2. Do you notice leaking urine when you sneeze, cough or exercise?
  3. Do you have trouble reaching the toilet in time?
  4. Do you use the bathroom more than 8 times during the day?
  5. Do you use the bathroom more than once at night?
  6. Have you ever lost bowel control?
  7. Do you have bowel urgency? Or constipation?
  8. Do you notice pain before, during or after urination?
  9. Do you notice pain before, during or after a bowel movement?
  10. Do you feel a bulge or protrusion in your genital area?
  11. Do you avoid activities because of any of these symptoms?
