We accept application forms October – December each year. All applications must be postmarked by December 31st. Applications postmarked after this date will not be accepted. See application, interview, selection and acceptance dates.
A $50.00 non-refundable application fee is required. Submit a personal check or money order payable to Aurora St. Luke’s Medical Center.

Admission policies
Our program does not practice open enrollment or advanced placement. The program does not accept transferring of credits from another ultrasound program or credits from experiential learning. There is no waiting list. View our admission policies for more information.
Applicants must achieve a minimum GPA on mandatory coursework as defined by the program.
Application requirements
Class size is limited, and not all applicants are admitted to the program. Applicants who are selected for interviews will be notified by email.
You must meet all the following application requirements. If you don’t, you will not be eligible for admission to the program.
All applicants must:
- Meet the program’s personal characteristic requirements.
- Comply with one of the program’s three prerequisite pathways. We encourage you to complete all prerequisites by the end of the enrollment period.
- Pathway 1 applicants must complete their allied health program prior to the start of the sonography program in the fall.
- Pathway 2 applicants must complete all program prerequisites prior to application.
- Pathway 3 applicants must complete all prerequisites and be in good standing prior to the start of the program.
- Complete a job shadowing experience.
- Each candidate will complete a minimum of four hours in cardiac AND a minimum of four hours in general/vascular.
- Acceptable sites for a job shadowing experience are a hospital or medical center.
- Submit a completed job shadow verification form with your application.
- Have current, Basic patient care experience. You must have the Verification of patient care form completed by your current employer and return the form with your application materials. Failure to submit the required documentation will result in an incomplete application.
- All applicants will submit an autobiography statement. The autobiography will be 1 - 2 pages in length describing why the field of sonography interests them. The statement will also include the applicant’s qualifications for becoming a Diagnostic Medical Sonographer. The statement will be typed, signed, and dated by the applicant to verify authenticity.
- Submitting transcripts
Official academic transcripts from post-secondary educational institutions are required to document completed prerequisites. To maintain eligibility, your official, sealed academic transcripts must be received and postmarked by the application deadline. Transcripts should be sent directly to our program by your educational institution.
Faxed, emailed or hand-delivered applications will not be accepted.
General eligibility
All required information must be submitted by the due date. If you miss a deadline, your application will be rejected. Official transcripts must verify all educational information provided. Any discrepancies will terminate your eligibility.
Carroll University provides a Direct Admit program with the Diagnostic Medical Sonography program. Applicants considered for the program will be required to pass a drug screen and physical exam.
