Program eligibility
- Completion of ACGME-accredited Cardiovascular Disease Fellowship
- Board eligible in Cardiovascular Disease at start of advanced fellowship training
Apply through ERAS
Applicants may apply to the Aurora Heart Failure & Transplant Fellowship program via the Electronic Residency Application System (ERAS). ERAS begins accepting applications in July. Our program excepts ERAS applications through December 31st of each academic year.
A completed application must include the following documents:
- ERAS common application form.
- Photo.
- Personal statement.
- Curriculum vitae (CV).
- USMLE/COMLEX scores (1,2,3 steps).
- Copy of ECFMG certificate (if applicable).
- Three letters of recommendation, one of which must be from your current or prior cardiology program director.
- If you’re a non-U.S. citizen, you’ll be required to provide a copy of your resident status if you are invited for an interview. Aurora Health Care does not sponsor H-category visas.
