PGY2 Ambulatory Care Pharmacy Residency
The PGY2 Ambulatory Care Pharmacy Residency program prepares you to specialize in advanced ambulatory patient-centered care through interdisciplinary collaboration. This program focuses on enhancing patient access and ambulatory services through integration within ambulatory clinics, population health, and transitions of care.
Rotation site
Rotations occur within the ambulatory clinics and outpatient pharmacy at Aurora St. Luke’s Medical Center. Residents serve the community and caregivers, as well as clinic and hospital discharge patients. You will have the opportunity to collaborate and build strong relationships with other hospital departments and clinics. Aurora clinics and pharmacies unify patient care by using the same electronic health record and prescription processing software.

Program highlights
The format of the residency is tailored to the interest, experience, aptitude and career goals of each resident.
Orientation and training: Computer systems, drug distribution, personnel, policies, workflow and department structure
Ambulatory Clinics: Providing direct patient care in an interdisciplinary team setting, and practicing under a collaborative practice agreement in select clinics*

- Abdominal Transplant Clinic
- Advanced Heart Failure Clinic*
- Anticoagulation Clinic*
- Endocrine Clinic*
- Malignant Hematology/Stem Cell Transplant Clinic
- Pulmonary Clinic*
Virtual Population Health*: Participating in MTM referrals, medication optimization programs, medication adherence outreach, and disease state management services.
Transitions of Care: Collaborating with patients, inpatient and outpatient providers and hospital personnel to assist in the transition of care process.
Medication Safety: Evaluating ambulatory clinic medication errors and workflow processes; providing education on medication safety to clinic personnel.
Outpatient Pharmacy Staffing: Staffing in the outpatient pharmacy to provide clinic and discharge prescription support.
Practice Management: Clinic budgeting, operations and personnel management, and development and implementation of ambulatory pharmacy services.
Year-long residency project: Conducting a longitudinal project focused on expansion or advancement of ambulatory pharmacy services.
Professional development: Learning experience projects, staffing, professional organizations, precepting, CE continuing education presentation, interviewing/recruiting.

Residency projects
Each pharmacy resident also completes a residency project. Pharmacy residency projects are practice-based and align with the department’s strategic plan. Residents formally present their projects at a statewide conference and receive the opportunity to publish their work.

Program interview
All pharmacy residency candidates must complete a virtual interview. Interview dates cannot be rescheduled.
