What does it mean to be in home hospice?
Being in home hospice means receiving compassionate, patient-centered care where you feel most comfortable. Aurora home hospice provides end-of-life support to patients and their families to help ensure comfort and peace during this time.
Starting Aurora home hospice care
More than 95% of people who choose hospice receive their care at home. If you and your doctor decide Aurora home hospice is right for you, you can expect:
- Assessment: A nurse evaluates your needs and your family’s needs for hospice care. Needs are then reassessed on an ongoing basis.
- Hospice visits: Your case manager nurse will visit as needed based on your condition, modifying the visit schedule as your needs change.
- 24/7 helpline: Our helpline answers questions related to hospice care and arranges further care when needed.
- Telehospice: Families using our telehospice or virtual health remote monitoring program complete a daily questionnaire, allowing telehospice nurses to monitor symptoms between visits.
Deciding it’s time for hospice care
Choosing hospice care is a very personal decision. Perhaps you no longer want treatment meant to provide a cure, or maybe your doctors believe such treatment won’t continue working.

- Have cancer
- Have heart disease
- Have pulmonary disease
- Have renal disease
- Want quality-of-life care and not life-prolonging or curative care
- Have a life expectancy of 6 months or less
