Refer a patient for home health services

Accessing home health services begins with a referral. You provide us with any needed instructions and information. We’ll ensure that people who meet home health services criteria get the compassionate care they need, when and where they need it.

Referring to home health services has never been so easy

Before ordering, check that the intended recipient meets the criteria for home health services. If someone is a candidate for home health services, as an Aurora provider you’ll order services through Epic.

To access the home health services order, first determine whether the person you want to help is currently in the inpatient or ambulatory setting:

• From the inpatient setting: Place home care orders as a new order for discharge.
• From the ambulatory setting: Place home care orders through a SmartSet called Home Care Services [1573].

Depending on what’s needed, select “Service to Home Care” (for home health, hospice, and infusion pharmacy) or “DME or respiratory therapy”. Once you make your selection, the corresponding order appears. The program will guide you through the order.

Tips for ordering home health services

Read this information before submitting your order:

• Service options: You can select multiple service options in each order.
• Referral priority: The program defaults to “routine.” If services are needed within 24 hours, select “urgent.”
• Resuming care: Submit a new referral when resuming home care. Make sure to note the resumption in the comment box. You must complete the face-to-face and home health services eligibility documentation again.
• Required documentation: Only nursing, physical therapy; and speech therapy require face-to-face documentation that includes the primary causal diagnosis. Uncheck the box for face-to-face documentation.

Refer someone for home health services

Place an order for home health services if you’re prescribing skilled nursing services, physical therapy; or speech therapy. Before referring someone for these services, check that the recipient meets the criteria for home health services.

When you are ready to refer to home health services, fax the required documentation to 414-290-2850. Your signed order must include:

• Cover sheet (noting the date of discharge, services needed and primary care physician)
• Home Care Referral Form [PDF]
• Medication list
• History and physical or discharge summary
• Physician Progress Note and primary causal diagnosis for Home Health Eligibility & Face-to-Face Encounter, to meet CMS regulations
• Most recent therapy notes, if applicable
• Special instructions

Order home medical equipment

Aurora Health at Home offers a wide range of home medical equipment. We have everything needed to stay safe at home, from assistive (or ambulatory) devices to respiratory equipment.

To place a home medical equipment order, call customer service at 414-329-5500, option 1. You may also fax orders to 414-327-6965 or send them via email. Please note:

• Customer service representatives are available from 8 a.m. to 6 p.m., Monday through Friday.
• We make every effort to process orders received during normal business hours within 24 hours.
• All orders are subject to benefit eligibility and insurance verification.
• Respiratory therapy may require a valid physician prescription.

Refer to hospice care

Hospice care is available in residences, at certain Aurora medical centers and at the Aurora Zilber Family Hospice. To inquire about hospice or refer someone, call 833-268-1268 or fax 414-385-8959. You can also learn more about working with families to begin hospice.

Refer to home infusion

We begin the process to deliver infusion therapy as soon as possible after referral, including insurance verification and training for the patient or family. For more information about referring someone to Aurora Health at Home infusion therapy, call 800-862-2201. Fax orders to 414-290-2850.
